Your First Visit

Patient Forms

When you arrive at Frontier Chiropractic, our Clinic Manager will greet you and welcome you as a member of our family.  Next it will be time to provide your photo ID and insurance card (if using insurance) and to fill out several documents including the Patient Intake Form (if not already submitted online), the Notice of Financial Responsibility, the HIPAA form and the Informed Consent Document. The Clinic Manager will guide you through this process and provide paper copies of any of the documents upon request.


Consultation

Upon completing your forms, the path to wellness begins! You will now  have a consultation with Dr. Corey Weinfurtner to review the information you provided on the Patient Intake Form in regard to your current health, any past illnesses or injuries, and of course - what brought you into Frontier Chiropractic for treatment. Dr. Corey will also discuss any health-related problems you may have, any health concerns you may have, and potential treatment options. Your first visit is designed for Dr. Corey to learn more about you, your condition and your expectations, to determine whether Chiropractic care will you meet your goals, and whether Chiropractic Treatment is appropriate for you.


Examination

After your consultation, Dr. Corey will perform a complete Chiropractic examination and physical assessment. This may include reflexive testing, your ability to turn and bend,  as well as other standard neurologic, orthopedic, postural, and physical examinations. All tests and assesments are done with your consent by way of the documents mentioned above and a brief one-on-one verbal check. 


Report of Findings

Once all intake information has been reviewed and discussed and all assessments and examinations have been performed, Dr. Corey will give you a verbal report of findings that will address the three common questions that the majority of patients have:
  1. Am I a patient candidate for Chiropractic care?
  2. What do you recommend that I do to to aid in my treatment and to improve my condition?
  3. How often will I receive treatment?
In addition, based on the information collected through your intake documents, the consultation and the initial examination, Dr. Corey will provide recommendations for treating your specific condition(s). If you have a condition that requires care from other providers, Dr. Corey will be very clear about it, and refer you to an appropriate physician who can treat you. If Dr. Corey believes that Chiropractic care, is appropriate for you and will improve your condition he recommend treatment and a wellness program.


Treatment

Patients most often receive their first treatment at the conclusion of the consultation and initial examination. In other words, you'll most often be able to receive treatment on your very first appointment! Treatment may include spinal adjustments, physical and rehabilitative therapies, and soft tissue therapies.  Treatment is interactive between Patient and Provider (Doctor, Chiropractic Assistant and/or Massage Therapist), therefore you are able to provide immediate feedback resulting in  a level of treatment you are most comfortable with.


Wellness Program

Upon the conclusion of the appointment, patients will be provided on a wellness program to be conducted at home. This may include ice or heat application instructions, pain management recommendations, instructions on the avoidance of certain sleeping/sitting/standing positions, work/physical fitness restrictions,  as well as rehabilitative exercises and/or stretches.  If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success.  Every body is unique and different and therefore every patient requires a unique and customized wellness plan.  The foundation of our overall wellness lapproach is maintaining  healthy posture and good spinal alignment, eating a healthy diet, taking supplements, keeping your weight under control and relieving stress.
